The proof of care
is as important as the care itself.
Nowhere in healthcare is proof more challenging than in behavioral health. Whether care is covered is subjective, adversarial, and varies from state to state. Providers speak clinical, while payors speak actuarial. Nobody built the layer in between.
Both founders saw that gap firsthand. Keshav spent years building regulated infrastructure at scale, from SwaggerHub to emergency-response systems, and witnessed the same failure firsthand himself. Moe, on the other hand, was seventeen when his mother was denied prior authorization for a heart condition. The care was appropriate, but the proof was lacking.
They met at a hackathon and built a care coordination tool that clinicians loved. However, because they were fee-for-service, which relies heavily on proof, many providers found that care wasn't the only bottleneck. Proving medical necessity was.
So Circle built the missing layer to address this gap by creating a shared set of expectations. We analyzed payor policies, state standards, accreditation rules, and each facility’s approval and denial history to develop this shared set of expectations. Then, we placed digital assistants at the points where the clinical record interacts with payors, such as charting, authorization, compliance, and claims.
This effectively closes a loop that healthcare has never experienced before. Payor expectations are integrated into care as it occurs, and proof of that care is provided before the claim is submitted. As a result, facilities are compensated for the work they already perform, and clinicians can focus on providing care to their clients.

Keshav co-founded Circle Health after more than a decade building infrastructure products in regulated industries, where he repeatedly encountered the same gap in his personal experience. While great care was being provided, providers struggled to demonstrate it to payors. He was the first product hire for Swagger/OpenAPI and later served as Head of Product & Design at Prepared. Under his leadership, Prepared scaled from a handful of 911 centers into a national public-safety platform before its acquisition by Axon. Keshav’s entrepreneurial journey began at the age of 19, where he used his earnings to fund a master’s degree in business and engineering at Dartmouth.

Moe co-founded Circle Health after experiencing firsthand the exorbitant costs of prior authorization. At the age of 17, his mother’s heart condition was denied coverage, compelling his family to make significant sacrifices. As a career software engineer, Moe has built infrastructure across healthcare and other highly regulated industries, including Walmart Pharma, Bausch Health, and Nutrien. At Nutrien, he developed platforms utilized by over 100,000 farmers worldwide. Today, he leads the technical infrastructure behind Circle, designed to securely scale sensitive clinical data.
